Overview
The Mediterranean diet is associated with a 47% reduction in GERD odds (OR=0.53) at the highest adherence level. Multiple complementary mechanisms contribute to symptom relief beyond simple acid suppression.
Mechanism
- Fiber as nitrite scavenger: Dietary fiber binds nitrite in the gastric lumen, reducing nitrosative stress at the gastroesophageal junction
- Antioxidant protection: Polyphenols and vitamins reduce reactive oxygen species (ROS) in esophageal mucosa
- Melatonin-rich foods: Nuts, tomatoes, and olive oil contain melatonin precursors that improve lower esophageal sphincter (LES) tone and sleep quality
Clinical Evidence
Cross-sectional study (n=5,141) demonstrated a dose-response relationship between Mediterranean diet adherence and GERD symptom reduction, with the highest adherence tertile showing OR=0.53 for GERD.
