Mediterranean Diet For GERD

Overview

The Mediterranean diet is associated with a 47% reduction in GERD odds (OR=0.53) at the highest adherence level. Multiple complementary mechanisms contribute to symptom relief beyond simple acid suppression.

Mechanism

  • Fiber as nitrite scavenger: Dietary fiber binds nitrite in the gastric lumen, reducing nitrosative stress at the gastroesophageal junction
  • Antioxidant protection: Polyphenols and vitamins reduce reactive oxygen species (ROS) in esophageal mucosa
  • Melatonin-rich foods: Nuts, tomatoes, and olive oil contain melatonin precursors that improve lower esophageal sphincter (LES) tone and sleep quality

Clinical Evidence

Cross-sectional study (n=5,141) demonstrated a dose-response relationship between Mediterranean diet adherence and GERD symptom reduction, with the highest adherence tertile showing OR=0.53 for GERD.

Advantages Over PPI Monotherapy

  • Addresses upstream drivers rather than suppressing acid alone
  • No risk of PPI-associated dysbiosis or rebound hypersecretion
  • Additional cardiovascular and metabolic benefits
