- Pilot studies demonstrate increased fecal Bifidobacterium and F. prausnitzii counts following prebiotic supplementation in CD patients
- FOS supplementation (15g/day) increased fecal bifidobacteria and improved dendritic cell IL-10 responses in active CD (small pilot, n=10)
- Resistant starch selectively increases Ruminococcus bromii (primary degrader) which cross-feeds butyrate producers
- Larger RCTs are needed — current evidence is limited to small pilot and case-series designs
Mechanism
- Selective feeding: Inulin and FOS are preferentially fermented by Bifidobacterium and cross-feeding partners, bypassing proteolytic species
- Butyrate restoration: Downstream butyrate production nourishes colonocytes, strengthens tight junctions, and reduces epithelial permeability
- Anti-inflammatory signaling: Butyrate inhibits NF-kB in intestinal epithelial cells and induces Treg differentiation via HDAC inhibition
Clinical Context
Caution in active flares: Prebiotic fiber may exacerbate symptoms during active inflammation due to gas production and osmotic effects. Best introduced during remission or mild disease with gradual dose escalation. Practitioners should monitor for tolerance and consider low-FODMAP transitioning protocols before introducing targeted prebiotics.
