Intervention Summary
Dietary fiber supplementation (resistant starch, inulin, arabinoxlan oligosaccharides, or whole-food fiber) to shift gut fermentation from proteolytic to saccharolytic, reducing the two key protein-bound uremic toxins (indoxyl sulfate and p-cresyl sulfate) that drive CKD progression. This is the only intervention with meta-analytic evidence for reducing both IS and PCS in CKD — probiotics/synbiotics do not achieve this.

Meta-Analysis (Yang 2021)
- Design: Systematic review and meta-analysis of 10 RCTs, n=292
- Indoxyl sulfate: Significantly reduced (SMD -0.55, 95% CI -1.04 to -0.07, P=0.03)
- p-Cresyl sulfate: Significantly reduced (SMD -0.47, 95% CI -0.82 to -0.13, P<0.01)
- BUN: Significantly reduced (SMD -0.31, P=0.03)
- Uric acid: Significantly reduced (SMD -0.60, P<0.01)
- IS reduction more pronounced in dialysis patients (P for interaction=0.03)

- Per 5g/day increase in total fiber: 11% reduced risk of incident CKD
- Legume fiber: 50% reduced risk per 5g/day (OR 0.50)

Mechanism
Fiber targets the core ecological disruption in CKD — the saccharolytic → proteolytic shift:
- Substrate competition: Fiber provides fermentable carbohydrate to saccharolytic bacteria, reducing the relative availability of amino acid substrates for proteolytic (uremic toxin-producing) species
- SCFA restoration: Feeds depleted butyrate producers (Faecalibacterium, Roseburia, R. bromii, Eubacterium rectale) to restore colonic butyrate
- Gut barrier repair: Butyrate nourishes colonocytes and strengthens tight junctions, reducing bacterial translocation and endotoxemia
- pH modulation: SCFA production lowers colonic pH, counteracting ammonia-mediated alkalinization from bacterial urease
Clinical Context — The CKD Fiber Paradox
CKD patients have the greatest need for dietary fiber but often eat the least. CKD dietary restrictions (potassium, phosphorus) inadvertently reduce fiber intake well below general population recommendations (carrero 2020 plant based diets ckd nature reviews). This creates an iatrogenic contribution to the proteolytic shift. Fiber supplementation should be considered a core CKD management strategy, not an optional adjunct.
Caution in advanced CKD: High-potassium fiber sources (bananas, potatoes, legumes) require monitoring in stages 4-5/dialysis. Low-potassium fiber sources (resistant starch, psyllium, inulin) can be used safely.
